Dermal Fillers in Istanbul: Natural Facial Rejuvenation Without Surgery

dermal fillers in istanbul

Our faces change with time. We lose volume in the cheeks and temples, lines appear around the mouth, and the jawline slowly becomes less defined. Dermal fillers are one of the simplest and most effective ways to restore this lost volume, soften lines and bring balance back to the face, without surgery and without downtime.

I am Op. Dr. Akın Şahin, a European Board certified ENT, head and neck and facial surgeon in Istanbul. As a facial surgeon, I plan every filler treatment with a detailed understanding of facial anatomy, blood vessels and proportions. My aim is never to change who you are, but to help you look rested, fresh and natural. In this guide, I explain what dermal fillers are, which areas can be treated, how the procedure works and what you can expect afterwards.

What Are Dermal Fillers?

Dermal fillers are gel-like substances injected under the skin to restore volume, smooth lines and enhance facial contours. The most commonly used fillers today are made of hyaluronic acid (HA), a substance that occurs naturally in our skin and helps it stay hydrated and plump. Because hyaluronic acid is already part of the body, it is very well tolerated, and it can be dissolved with an enzyme called hyaluronidase if needed.

Other types of fillers include calcium hydroxylapatite and poly-L-lactic acid. These are often called “biostimulators” because they also encourage the skin to produce its own collagen. The right product depends on the area being treated, the thickness of your skin and the result you would like to achieve. For most of my patients, I prefer high-quality hyaluronic acid fillers because they look natural, feel soft and are reversible.

Who Can Have Dermal Fillers?

Facial fillers are suitable for most healthy adults who would like to improve the shape of their face or reduce the signs of ageing. Common reasons patients come to me include:

  • Thin or asymmetric lips
  • Deep smile lines (nasolabial folds) and marionette lines
  • Flat cheekbones or loss of volume in the mid-face
  • A weak or retruded chin and an undefined jawline
  • Hollow under-eyes and dark circles
  • Sunken temples
  • Small bumps or irregularities on the nose

Fillers are not suitable during pregnancy or breastfeeding, in the presence of active skin infection in the treatment area, or in people with certain autoimmune diseases or known allergies to filler ingredients. During your consultation, I review your medical history to make sure treatment is safe for you.

How Are Dermal Fillers Performed?

Every treatment begins with a facial analysis. I look at your face from the front and in profile, assess its proportions and listen carefully to what bothers you. Together we decide which areas to treat and how much product is needed. Photographs are taken before treatment so we can compare results.

The skin is cleaned and, where needed, a numbing cream is applied. Most modern fillers also contain lidocaine, a local anaesthetic, which makes the injection much more comfortable. Depending on the area, I use either a fine needle or a blunt-tipped cannula. Cannulas reduce the risk of bruising and of injecting into a blood vessel, which is especially important in high-risk areas such as the nose and under-eyes.

The procedure usually takes 10–15 minutes for a single area and up to 30–40 minutes for multiple areas. Results are visible immediately, and you can return to your daily routine the same day.

Which Areas Can Be Treated With Fillers?

1. Lip Fillers

Lip fillers add volume, define the lip border and correct asymmetry. A natural result usually requires 0.5–1 ml of filler. You can learn more on my lip fillers page.

2. Nasolabial Fold Fillers

The nasolabial folds are the lines that run from the side of the nose to the corners of the mouth. Fillers soften these lines and make the face look less tired. Often, restoring volume in the cheeks first gives a more natural improvement than filling the fold directly.

3. Cheekbone Fillers

Cheek fillers lift the mid-face, restore youthful contours and create more defined cheekbones. Because the cheeks support the lower face, this treatment also softens smile lines and jowls.

4. Chin Fillers

A small or retruded chin can make the nose look larger and the face look unbalanced. Chin filler brings the chin forward and improves the profile without surgery.

5. Jawline Fillers

Jawline filler creates a sharper, more defined lower face. It is popular with both women, who usually prefer a soft and elegant contour, and men, who often want a stronger, more angular jaw.

6. Marionette Line Fillers

Marionette lines run downwards from the corners of the mouth and can give the face a sad expression. Small amounts of filler lift the corners of the mouth and soften these lines.

7. Under-Eye (Tear Trough) Fillers

Hollow under-eyes make the face look tired even after a good night’s sleep. A carefully placed, soft hyaluronic acid filler reduces the hollowness and shadows. This is a delicate area that requires experience, and I always use a cannula here.

8. Temple Fillers

With age, the temples become hollow and the upper face loses its smooth outline. Temple fillers restore this volume and give a fresher appearance to the whole face.

What Are the Advantages of Dermal Fillers?

  • No surgery: Fillers are injected, so there are no incisions and no general anaesthesia.
  • Quick treatment: Most areas take 10–30 minutes.
  • Immediate results: You see the improvement as soon as the treatment is finished.
  • Little or no downtime: You can return to work the same day.
  • Natural and adjustable: Treatment can be built up gradually, and hyaluronic acid fillers can be dissolved if needed.
  • Long-lasting: Results last between 6 and 24 months depending on the area and product.

Aftercare: What Should You Do After Filler Treatment?

Correct aftercare helps your result settle well and reduces the risk of side effects:

  • Avoid touching, rubbing or massaging the treated area for the first 24 hours, unless I advise otherwise.
  • Avoid saunas, steam rooms, hammams and very hot showers for 48 hours.
  • Avoid strenuous exercise for 24–48 hours.
  • Do not apply make-up for 12–24 hours.
  • Sleep on your back with your head slightly raised for the first few nights.
  • Avoid alcohol for 24 hours, as it can increase swelling and bruising.

What Is Normal After Fillers?

Some mild reactions are normal after filler injections and settle on their own:

  • Swelling: Usually most noticeable in the first 2–3 days, especially in the lips.
  • Bruising: Small bruises may appear at injection points and fade within 5–7 days.
  • Tenderness: The area may feel slightly sore for a few days.
  • Firmness: The filler may feel firm at first and softens as it integrates with your tissue.

If you notice severe or increasing pain, skin colour changes such as whitening or a dusky appearance, or any change in your vision, contact me immediately. These are rare but require urgent treatment.

Who Should Not Have Fillers?

Dermal fillers are not recommended for:

  • Pregnant or breastfeeding women
  • People with an active infection, cold sore or inflammation in the treatment area
  • People with a known allergy to any filler ingredient
  • People with certain uncontrolled autoimmune or bleeding disorders
  • Anyone with unrealistic expectations about what fillers can achieve

If you take blood-thinning medication or supplements, please tell me during your consultation.

How Long Do Dermal Fillers Last?

The longevity of hyaluronic acid fillers depends on the area, the product and your metabolism:

  • Lips: about 6–12 months
  • Cheeks, chin and jawline: about 12–18 months, sometimes longer
  • Under-eyes: often 12 months or more
  • Biostimulating fillers: up to 18–24 months

Regular exercise, a fast metabolism and sun exposure can shorten the effect. Many patients choose small top-up treatments to maintain their result.

Dermal Fillers or Anti-Wrinkle Injections?

Patients often confuse fillers with anti-wrinkle injections, but they work in completely different ways. Dermal fillers add volume: they fill hollows, lift sagging areas and create contour. Anti-wrinkle injections relax the muscles that cause expression lines, such as frown lines, forehead lines and crow’s feet.

As a general rule, lines that appear only when you move your face (dynamic lines) respond best to anti-wrinkle injections, while lines and hollows that are visible at rest respond best to fillers. Many patients benefit from a combination of both. During your consultation, I explain which treatment suits each area of your face.

Why Choose a Facial Surgeon for Fillers?

Filler injections may look simple, but the face contains many blood vessels and nerves, and their position varies from person to person. Knowing exactly where and how deep to inject is what separates a safe, natural result from a complication or an overfilled look.

As an ENT and facial surgeon, I work with facial anatomy every day in the operating room. This experience helps me plan treatments that respect your natural proportions and recognise when surgery, rather than filler, is the better solution. If a complication ever occurs, I am trained to recognise and manage it immediately. I use only original, certified products from established manufacturers, and I open every product in front of the patient.

Frequently Asked Questions (FAQ)

Are dermal fillers painful?

Most patients describe only mild discomfort. Numbing cream and the lidocaine contained in most fillers make the treatment very tolerable.

Will I look natural?

Yes. My approach is to restore balance rather than to overfill. I prefer to treat in small amounts and, if needed, add more at a follow-up visit.

Can fillers be reversed?

Hyaluronic acid fillers can be dissolved with an enzyme called hyaluronidase. This is one of the reasons I prefer them for most areas.

How soon will I see results?

Results are visible immediately. The final result is seen after about two weeks, when swelling has gone down and the filler has settled.

How much do dermal fillers cost?

The cost depends on the area and the amount of filler needed. You can find detailed information on my lip filler cost and jawline and chin filler cost pages, or contact me for a personal treatment plan.

Can fillers be combined with other treatments?

Yes. Fillers are often combined with anti-wrinkle injections, skin boosters or Endolift for a complete facial rejuvenation.
