The honest answer is that there is no single best facelift for everyone. There is only the best technique for your face, your degree of ageing and your goals. In this article, I explain the differences between the deep plane facelift, the SMAS facelift and the mini facelift, so you can understand what your surgeon is recommending and why.

Why the Technique Matters

The face ages in layers. The skin loses elasticity, the fat pads shrink and descend, and the ligaments that anchor the face weaken. Under the skin and fat lies the SMAS (superficial musculoaponeurotic system), a supportive layer connected to the facial muscles. Modern facelifts work on this deeper layer rather than pulling the skin alone, which is why they look more natural and last longer than older skin-only lifts.

The main difference between techniques is how deeply the surgeon works and which structures are released and repositioned.

The Mini Facelift

A mini facelift uses shorter incisions, usually around the front of the ear, and lifts the SMAS in a limited area of the lower face and jawline.

Best for: Patients, often in their late thirties to early fifties, with early jowls and mild loss of jawline definition, and little loose skin in the neck.

Advantages:

Shorter operation and shorter recovery

Smaller incisions

Can sometimes be performed under sedation

Limitations: It does not treat the neck well, has a limited effect on the mid-face and usually does not last as long as a full facelift. If you have significant neck laxity, a mini facelift will probably disappoint you.

The SMAS Facelift

In an SMAS facelift, the skin is lifted from the SMAS, and the SMAS is then tightened, folded (plication) or partially removed and repositioned (SMASectomy). The skin and SMAS are therefore handled as two separate layers.

Best for: Patients with moderate sagging of the lower face and jowls, often combined with a neck lift.

Advantages:

Reliable, well-established technique

Good correction of jowls and jawline

Can be combined with neck tightening

Limitations: Its effect on the mid-face and nasolabial folds is more limited, because the ligaments holding the cheek tissues are not fully released.

The Deep Plane Facelift

In a deep plane facelift, the surgeon works beneath the SMAS and releases the key retaining ligaments of the face. The skin, fat and SMAS are lifted together as a single unit, rather than separately.

Best for: Patients with moderate to advanced ageing of the mid-face, jowls and neck, who want a natural, long-lasting result.

Advantages:

Lifts the cheek fat pad, softening the nasolabial folds and restoring mid-face volume

Gives a very clean jawline and strong correction of jowls

Tension is carried by the deep tissues, not the skin, so results look natural and scars heal better

The skin keeps its blood supply attached

Results generally last longer

Limitations: It is technically more demanding, takes longer and requires excellent knowledge of facial nerve anatomy, because the dissection is close to the branches of the facial nerve. It is also at the upper end of the price range.

Side-by-Side Comparison

Area treated: Mini facelift: lower face and jawline. SMAS facelift: lower face, jawline, often neck. Deep plane facelift: mid-face, lower face and neck.

Incisions: Mini: shorter, around the ear. SMAS and deep plane: in front of and behind the ear, into the hairline.

Recovery to social life: Mini: about 7–10 days. SMAS and deep plane: about 2–3 weeks.

Longevity: Mini: shorter. SMAS: long-lasting. Deep plane: generally the longest-lasting.

Natural look: All can look natural in experienced hands; deep plane is especially strong for the mid-face.

What About the Neck?

The neck often shows ageing earlier than the face. A facelift alone improves the jawline and upper neck, but if you have significant loose skin, fat or vertical bands in the neck, a neck lift is usually needed. This involves a small incision under the chin to tighten the platysma muscle and remove excess fat. I explain the combination on my face lift and neck lift page. If the main problem is fat under the chin with good skin, double chin removal may be enough.

How I Choose the Technique

During your consultation, I examine your face at rest and in motion, assess the position of your cheek fat, the depth of your folds, the jowls, your neck and your skin quality. I also ask what bothers you most. Some patients are mainly concerned about the jawline; others about tired cheeks or the neck.

Based on this, I recommend the technique that gives you the best result with the least necessary surgery. Sometimes that is a deep plane facelift with a neck lift; sometimes it is a mini facelift; and sometimes surgery is not yet needed, and I recommend non-surgical options such as Endolift or a liquid facelift instead.

How Does the Technique Affect the Cost?

The technique is one of the main factors in the price. At my practice in Istanbul, the total facelift cost in Turkey ranges from $4,500 for a mini facelift up to $9,500 for a deep plane facelift with neck lift, including hospital, hotel and transfers. You can find the details on my facelift cost in Turkey page.

Frequently Asked Questions

Is a deep plane facelift always better?

Not always. It is the most powerful technique for mid-face and jowl ageing, but a patient with early changes may achieve an excellent result with a less extensive procedure.

Is a deep plane facelift riskier?

It requires greater technical skill because the dissection is closer to the facial nerve branches. In experienced hands, the risk of permanent nerve injury is very low.

Does a mini facelift leave smaller scars?

Yes. Its incisions are shorter, but the difference in visible scarring is small when any facelift is performed well.

How long does each facelift last?

A mini facelift usually lasts several years. SMAS and deep plane facelifts commonly last 10–15 years, with deep plane techniques generally lasting the longest.

Deep Plane vs SMAS vs Mini Facelift: Which Is Right for You? “Which facelift is the best?” is one of the most common questions I hear.
